To validate your identity, please enter the following information.

File Number (required)

File Number is the 8-digit number found on the top-right corner of your Mandatory Jury Duty Eligibility Form or covering letter.

Postal Code (required)

The postal code of the address as it appears on the Mandatory Jury Duty Eligibility Form. Please enter the postal code using spaces, e.g. X1X 1X1.

Year of Birth (required)

Enter all four digits of your Year of Birth (YOB). For example, if your YOB is 1977, enter 1977.

Important Information:

Ontario law requires you to complete and return the Mandatory Jury Eligibility Form within 30 days of receipt. You may complete the form either online or using the paper form you received. Do not do both.

If you require or prefer an accessible format, please contact the Provincial Jury Centre within 10 days of receipt of your jury form. The Provincial Jury Centre is open Monday to Friday from 8:30 am to 5:00 pm EST.

If you have moved or your address is incorrect, use your previous/incorrect postal code to access the portal. You can update your address through this portal, once you have completed the Mandatory Jury Eligibility Form.

You are only required to complete this form. If you are selected for jury duty, you will receive a summons notifying you when and where to appear.
