File Number (required)

File Number is the 8-digit number found on the top-right corner of the letter you received.

Postal Code (required)

The postal code of your address as it appears on the letter you received.

Year of Birth (required)

Enter all four digits of your Year of Birth (YOB). For example, if your YOB is 1977, enter 1977.

Important Information:

Ontario law requires you to complete the Mandatory Jury Eligibility Form within 30 days of receipt.

If you require a paper format, please contact the Provincial Jury Centre.

If you require an accessible format, please contact the Provincial Jury Centre within 10 days of receiving the letter. The Provincial Jury Centre is open Monday to Friday from 8:30 am to 5:00 pm EST.
